This guide contains detailed steps on how to add eForms to your account using either the Classic or Juno UI.

 

eForms can be added to Juno EMR either as zip files (with all the components of the eForm) or as separate file components (image files and HTML/ Javascript code). There are two separate ways to add the different file types to your account, and will be done in the admin panel.

 

In the Classic UI, the Admin Panel can be accessed by clicking on the "Administration" link at the top of the schedule page.

To access the Admin panel in Juno UI, click on “Admin” at the top of the schedule pageThe Admin tab may be hidden under the “More” option if you are using Juno with a smaller screen display, click on “More”, then click on “Admin” from the dropdown.

Note that eForms can only be added by users with admin access.

 

Adding a zip file

  1. In the Admin panel, you can either: 

    • click on the "Manage eForms" tile to the right, or

  • click on the "Forms/eForms" tab on the left then click on the "Manage eForms" link from the dropdown.

  1. Click on the "Import" button on the left of the eForm Library page (second button from the left).

  2. Click on the "Browse" button, to find and select the zip folder from where it's saved.

  3. After selecting the zip folder, click the "Import" button to the right of the file name.

Once the zip folder is imported, the eForm should be listed in the eForm library (on the same page).

Adding Separate files

If you have separate image and HTML/ Javascript code files, you'll need to add the files separately to Juno EMR in order for the eForm to load both components properly. 

 

Uploading the image file(s)

  1. Open the Admin panel.

  2. To access the Image Library, you can either;

    • click on the "Forms/eForms" tab on the left, then click on the "Upload an Image" link from the dropdown, or

    • click on the "Forms/eForms" tab on the left, then click on the "Manage eForms" link from the dropdown and click on the "image library" link on the top of the 'Manage eForms' page (last link from the left).

  3. Click on the "Browse" button to find and select the image file.

  4. After selecting your image file, click the “Upload” button to the right of the file name.

You can only upload one image file at a time. For eForms with multiple pages, you'll need to repeat the steps for each image file that represents a separate page of the form.

Uploading the HTML/Javascript code files

  1. Open the Admin panel.

  2. In the Admin panel, you can either: 

    1. click on the "Manage eForms" tile to the right, or 

    2. click on the "Forms/eForms" tab on the left then click on the "Manage eForms" link from the dropdown.

  3. Click on the "Upload" button on the left of the eForm Library page (first button from the left),

  4. Click on the "Browse" button on the right to find and select the file.

  5. Give the eForm a name using the ‘eForm name’ field and add any additional information to the ‘Additional Information’ field. If no name is given, you will receive an error.

  6. Enabling the; 

    1. "Show Only Latest revision of eForm Template" will prevent multiple copies of the eForm template from displaying in the eForms section of an eChart.

    2. "Show Only Latest revision of eForm Instance" ensures that only the latest saved copy of the form is displayed under the eForms section of the eChart.

  7. Click the "Upload" button underneath the eForm name field on the left. You should see the eForm listed in the eForm library.

When you click on the eForm name on the list of eForms, it should open up and the image files that you uploaded should get pulled directly into the eForm. If you see a broken image icon, it means that either the image file doesn't exist in your account yet, or that the image file name in the HTML does not match the image file name of the image itself. You can confirm this by clicking the "Edit" icon(first icon in the Action column) and searching for the image name in the HTML file to make sure it's correct.

 

You can also create a zip folder with all the file components by clicking on the "Export" icon(second icon in the Action column). You can then send this zip folder to other colleagues.
