From encounter notes to requisitions, from measurement tracking to prescriptions… The medical chart contains all the information you need! The best place to start is the Summary tab of the patient record.


To access the Summary page, you can…

  • Use the search tool, then navigate to the ‘Summary’ tab.
  • Click on the ‘E’ from an appointment on the main schedule page.
  • Click on an appointment on the sidebar schedule.

 

The summary page is divided into three sections: the left column, the center, and the right column. Here is an example:



 

The left column contains information about…

  • Preventions – Vaccination and preventive exams such as PAPs, mammograms or colonoscopy.
  • Medications – List of medications; includes active medications and long term medications that have expired.
  • Other Meds – Other medications such as natural remedies, or medications prescribed by other providers that you want to keep track of without entering them in the medications section.
  • Cumulative Patient Profile (CPP) – Details pertaining to the patient’s history and current situation. This includes the following sections:
    • Ongoing Concerns
    • Medical History
    • Social History
    • Family History
    • Reminders
    • Risk Factors
  • Disease Registry – Coded diagnostics saved in the chart.
  • Allergies – List of allergies.
  • Forms – Requisitions, sick notes, calculators… There are lots of options in this section!

The central section on the Summary page allows you to browse through encounter notes. The most recent entries are at the top. Scroll down to view more encounter notes. This section will also show CPP entries, eForms saves, documents assigned to the chart and tickler notes. These entries can be identified by looking at the color of the side bar on the left of each encounter note:

  • Regular encounter notes have a grey bar
  • CPP notes have a maroon bar
  • eForms have a green bar
  • Documents have a blue bar
  • Tickler notes have an orange bar

The tracker tab within the central section allows you to access the health tracker tool within the summary page itself.


The right column, Incoming, includes a list of all reports and documents associated with this patient. You can find more
